Federal form 940, state FUTA credit certification

To obtain Federal Form 940 information:

1. Log in to your account at www.uimn.org
2. On My Home Page, click Payment Information.
3. Click FUTA Credit.
4. Select the year for which you want the FUTA (Federal Unemployment Tax Act) information from the drop down menu.
5. Click Search.
6. Review the FUTA Credit Search Results, and then click Next.

NOTE: The information provided can be used to complete the Federal Form 940, or to satisfy an IRS request for state unemployment tax payment certification. Your 4th quarter wage report must be filed and accepted and your 4th quarter tax payment posted in order for this information to be accurate for the calendar year.

7. Click the checkbox(s) for where you want the submission information sent to. You may select both options:
   
   
   • Mail to IRS is the default address for IRS Certification.
   • Mail to other, include the address where the FUTA Credit information is to be mailed.
     
     
8. Click Submit.
